if we already set immutable policy on azure blob storage, should we need to tick option 'make recent backup immutable' on VBR?

What immutable policy?
For immutable Azure Blob object storage, please follow our kb article on how to configure your Azure Storage container:
https://www.veeam.com/kb4416
Then you can add Azure Blob to the backup server. And yes, you must enable 'make recent backup immutable' or you cannot add it.

What immutable policy that i mean is, on azure container we can make immutable storage policy (can use time base or legal hold). Shpuld we set this poloicy on azure side?

No. It would not be supported to use such policies outside of our product. The backup server would not be aware which blobs are immutable and which are not. It will lead to technical issues.
Please make sure to use the latest version of Veeam Backup & Replication v12. We support immutability on Azure Blob starting with version 12. Configure your Azure Storage Container accordingly to the KB article I have provided.
Then you can add your Azure Storage container to veeam and tick 'make recent backup immutable'.
The backup server will then care to specify the immutably period for each uploaded object.

Is VBR immutable setting will reflected to the Azure? I mean if on the VBR the immutable already enable, there is a possibility user can modify using azure portal?

No, a user or administrator can not remove those data which is protected with immutability.
But you can stop paying your bills in Azure, then Microsoft will surely delete that data.
