I have a feeling that this tape was somehow added to several media pools, which leads to barcode conflict.
Feel free to open support ticket - I'm sure support engineer will be able to pinpoint the solution.

Ahh so proper procedure may be removing from a media pool before adding to another eh?

Yes, remove tape from an old one.

What makes me curious about this case is that UI does not allow such thing to happen - we do not display tapes which are already added into some Media Pool in the "Add tape" step.
If you can reproduce your path to make this error happen, I will be happy to fix the behavior.

ill look as soon as I can. I'm more worried about restoring a folder from a vmware backup which has a bitlockered volume at this point. lol

If volume is encrypted at OS level, File-Level Restore won't work - that's the main idea of OS level encryption
You can run Instant Recovery and retrieve required files from a "staged" VM in this case.
