what better option to protect a physical server with SQL and iSCSI disk?
What is the best practice?
What care do we need to take?
If it is necessary to use an agent, will it be necessary to restart the physical server?

As you did not mention any cluster technology, I have no "best practices" in mind. It just works.
Yes, you need to reboot for the change block tracking (CBT) driver.
