In Veeam Backup for M365, is it possible to restore only permissions of a SharePoint online site using the Veeam Explorer for SharePoint? When selecting the restore permission checkbox, it seems the "Changed items" or "Missing items" needs to be checked.

Permissions-only restore is not supported. They will be restored along with the restored items (files, sites, etc).

You might want to look at Security Explorer from Quest. I use it for NTFS Permissions backup and restore and it does have SharePoint as an option, although I'm not sure if it works with 365 or if it's only for on-prem.
It might be worth looking into.
