This new Veeam Backup for Azure ONLY works with an appliance running IN a Azure VM, right? There's no option to have an on prem Veeam server reach out and back it up to a local repository, right? Just thinking through the actual costs, which (assuming you have to run a VM in Azure for the product) is like $75/mo for a small instance... I think?

Correct, you can deploy it from the Azure marketplace and get started. There is no on-premises version. Regarding the size of the instance, it is by default deployed with the minimal requirements to assure it works fine.

If you want to get Azure backups back on-prem, you can use the external repository to do a backup copy or restore it from an external repository.

Are there specific scenario's you are looking into, or need? John_wood is right with the external repository, but we are looking at a few different paths to work together with on-premises Veeam software.

However, we are using native cloud capabilities, so we will always need something running in the cloud to be able to do that.

I was asking the question to make sure there wasn't a way to use the Azure backup with already-owned on-prem Veeam backup servers/repositories instead of having to pay MS for the Azure VM usage perpetually. The method of operation you describe to run on premise and pull back backups of out of Azure does have Azure costs associated with data egress. You may find these can be expensive, so the decision is not just 'run a backup VM in Azure vs. run it on premise'. There is also similar tradeoffs in restore time / restore performance if data has to be moved more.
