Just setting up our VBSF. Does the Callback URL need to be accessible from outside our org?

No, it doesn't. In fact, it also works with internal IPs (192.168.x.x, 172.16.x.x, 10.x.x.x). Also, make sure you got the ports sorted for the management server as per https://helpcenter.veeam.com/docs/backu ... tml?ver=20
