Forever forward incremental .vbk space reduction
With forever forward incrementals, does removing VM's from the backup ever reduce the size of the .vbk? My understanding was that this only occurred if you ran an active full.

Your understanding is correct: after the amount of days specified for deleted VMs retention, blocks belonging to the deleted VMs in VBK are marked as free and then can be re-used by the new data. To reclaim the space, you need to perform an active full.
