What brand and version of SQL is installed with V12 new installation? Cannot seem to find the answer in the release notes or install instructions.

If you want to use MSSQL, you must deploy the MSSQL by yourself.

Yes. It is required.
You can always deploy your own staging server or use one from your production network if you have free capacities.

How do you tell Veeam to use a MSSQL server from another server that does not have Veeam installed on it? I bring this up because in another thread, the solution to object level restores on a database that is over the MSSQL Express limit of 10gig is to install SQL Standard.

You can specify the staging server within Veeam Explorer for Microsoft SQL server:
