I understand MS WSUS runs a DB and that it can be either Windows Internal DB or MS SQL. If the 1st one, how will Veeam B&R v6 handle it ? Should Application Aware backup be activated ?

As far as I’m concerned, Windows Internal Database is just another derivative of SQL Server Express 2005(without a user interface or tools).
So, VB&R will treat it as typical SQL-server without any specific issues.
And yes, application-aware image processing option should be activated. Since this functionality guarantees backup transactional consistency of a VM running VSS-aware applications (such as Active Directory, Microsoft SQL, Microsoft Exchange, SharePoint). More information regarding application-aware image processing can be found in User Guide (p.35).
