What are the prerequisites for the EC2 proxy appliance or the cause of this error?

Hello,
you need to have permissions to create and and run the EC2 proxy appliance. Requirements are described in the user guide (AWS IAM User Permissions)

Which type of size (a1, t2,...) did u pick to deploy the appliance? This could cause the issue as well.
