I have just signed up a new Office 365 backup client. They have several hundred mailboxes but want me to exclude dozens from the backup.
What is the best way to do this? Just exclude each user one-by-one or can I make them all the member of the same distribution group and exclude the group thereby excluding all users?

Yes you can exclude a group (I'm using a security group).
If you're licensed for dynamic groups you can use that for your include (e.g. I back up all non licensed users that are not disabled) but still have the manually populated exclude group for a few exceptions.
