Sorry for the, probably, stupid question... but suppose to have an exchange on-prem server....veeam gives to me 3 backup options:
- if it is a virtual machine I can simply use VBR + VEX
- if it is a physical machine: VeeamAgent + VEX
- in both cases: VBO + VEX
Its it correct? Are there any differences in terms of functionality? Or it is just a design decision?

You are correct you can use multiple solutions. The reason why you can also backup on-premises Exchange via VBO is for those who are running in a Hybrid environment.
If you are running a virtual exchange, the most easy way to back it up is to use VBR.

Maybe a bit additional information (and to prove it is not a stupid question
)
When you use option 1, you are going to do an image-level backup which includes the entire server, including the installation, configuration and so on
When you use option 2, you can have the same as a Bare Metal Recovery solution, or additionally deselect some volumes
When you use option 3, you are protecting data only! No configuration, no binaries...
