Enterprise Manager data collection error on 1 of 4 servers

I am able to add server to EM but cannot perform database replication or catalog replication of this backup server.

Database Replication results in this Error: Cannot insert duplicate key row in object 'dbo.C.Backup.Model.GuestDatabase' with unique index 'PK_C.Backup.Model.GuestDatabase_id'. The duplicate key value is (898f0c2a-0857-481a-a50d-ef57858006e2). The statement has been terminated.

Catalog Replication results: Failed to replicate catalog data from the server "Server.Name.edu". No connection could be made because the target machine actively refused it 127.0.0.1:9393

Also: This may be unrelated, but since adding this server, EM is unable to perform catalog replication on the other 3 backup servers with the same error, which was working prior to adding this server.

Hi Jeffrey, looks to be an internal database issue, support should be able to track it down, so please continue working with them.
