VMware tech support determined that there is an issue with the Inventory Service DB and the recommendation is to reinstall or re-initialize the inventory service DB.
What issues would I have with my backups and replications if I follow their recommendation?

Paul, if I'm understanding correctly, this operation will cause VM moref IDs change, which will result in Veeam B&R treating them as completely new VMs. In this case you would need to map the jobs to existing backups/replicas to continue incremental chains.
